All alumni who graduated from the Faculty of Policy Management, Faculty of Environmental and Information Studies or Graduate School of Media and Governance, and wish to obtain official academic certificates must make a request to the Academic Affairs Office, Shonan Fujisawa Campus, Keio University.

To obtain certificates, an alumnus/alumna or an authorized proxy must either
・ hand in documents to the Academic Affairs Office, SFC or
・ send documents by post to the address provided at the bottom of this page.

Any request made through telephone, fax, or e-mail will not be accepted.

Because of the privacy policy, the University needs to confirm that the request  is made by the alumnus/alumna in question. Therefore, the University requires an alumnus/alumna to present his/her official identification. A proxy requesting a certificate must present his/her official identification, a copy of an official identification of the alumnus/alumna and a ‘Letter of Authorization’ signed by the alumnus/alumna. A proxy must be someone in Japan who can request and receive certificates on behalf of the alumnus/alumna.

Certificates are issued only when all necessary information and documents have been submitted and verified.  The office does not accept any changes in the content of the request that has already been submitted.  Alumni must decide the number of copies and category of certificate that they wish to request before submitting required documents.  Issued certificates are not refundable.

Office Hours

Monday through Friday, 9:15 a.m. - 4:50 p.m.

Certificates cannot be issued on Saturdays, Sundays, national holidays, school holidays (January 10 ) and during office closing periods in summer and winter breaks. 

During the following period, certificates are not available to be issued for summer break reason. Certificates cannot be issued from
August 8 (Tuesday) 4:30PM - August 17 (Thursday) 9:15AM

During the following period, certificates are not available to be issued for maintenance reason. Certificates cannot be issued from
September 20 (Wednesday) 4:30PM - September 22 (Friday) 9:15AM

Information Required for Issuance

All alumni requesting a certificate are required to provide the following information to the Academic Affairs Office:
 

  • Student ID number for each undergraduate and graduate program (if you do not remember, leave it blank.)
  • Name in Chinese characters or katakana (and Roman alphabet if an English certificate is required)
  • Date of birth (and gender if an English certificate is required)
  • Month and year of graduation/completion/withdrawal
  • Name and language of certificate (English or Japanese)
  • Number of copies
  • Request to seal certificates (if necessary, provide concrete instructions.)
  • Present mailing address, email address, daytime telephone and cell phone numbers

Request in Person or through a Proxy at the Academic Affairs Office Counter

Request by Alumni

All alumni must present an official identification at the Academic Affairs Office counter to obtain certificates. The University accepts driver’s license, passport, health insurance card, the Certificate of Alien Registration, and Basic Resident Register card (with a photo). Resident cards, employee ID cards and student ID cards of other universities are not accepted. After receiving a request form, all alumni should fill out the form and submit it to the office. If alumni wish to receive their certificates by mail, they must also submit a self-addressed stamped envelope with stamps.

Request through a Proxy

To obtain a request form, the proxy must present an official identification, a copy of an official identification of the alumnus/alumna and a ‘Letter of Authorization’ signed by the alumnus/alumna at the Academic Affairs Office counter to certificates. After receiving a request form, the proxy should then fill it out and submit it to the office.
The format of the ‘Letter of Authorization’ is not specified, but it must clearly state the alumnus’s/alumna’s intention of authorizing the task to the proxy.

Request via Mail from within Japan

All alumni or their authorized proxy must send the following documents to the Academic Affairs Office:

1. A Copy of Alumni’s Official Identification

The University accepts only driver’s license, passport, health insurance card, the Certificate of Alien Registration (copy of both sides) and Basic Resident Register card (with a photo). Resident cards, employee ID cards and student ID cards of other universities will not be accepted.

2. Self-addressed Stamped Envelope

・A return-mail envelope must have the address of the alumnus/alumna or the proxy.  The university does not send certificates to alumni in care of a third party.
・All certificates are issued in A-4 size.
・The size of an envelope for a sealed certificate is 235×120 mm (長3 Policy Style).
・A return-mail envelope must be 240×190 mm (角5 Catalog Style) or comparable size when requesting sealed certificates or AUD Shutoku Shomeisho (AUD取得証明書).

3. Issuance Fee

All alumni or their proxies should enclose a money order (郵便定額小為替: yubin teigaku kogawase issued at any post office in Japan.  Yubin teigaku kogawase is only accepted in this case) as an issuance fee, or send cash by registered mail.  The name of the recipient or yubin teigaku kogawase should be Keio University Shonan Fujisawa Office.

How To Pay

There are 2 payment methods of fees for issuance of certificates. Please select from below.

  1. Someone residing in Japan pays on your behalf as proxy. (Domestic transaction in Japan, either by registered mail for cash or by postal money order). Please make sure to clearly indicate the following items in the extra space on the Academic Certificates for Alumni Request Form: 1) name and contact information (address and telephone number) of the proxy residing in Japan, and 2) the foreign address (including telephone number) where the certificate is to be sent to.
  2.  Payment by credit card. Please make the payment through the external link Credit Card Payment Website.

Note:

  1. For issuance of certificates, applications and payments are made to different offices/campuses based on your affiliated faculty/graduate school or course. Please refer to the chart on the Credit Card Payment System: School Information page. If you wish to have multiple certificates issued and sent to you from more than one campus, you must apply separately to each campus, stating how many copies you require.
  2. Each application paid for by credit card will incur a certificate issuing fee and postage, as well as the following service fees:
       ① Online service fee (500 yen)
            ② Credit card administrative fee
        (The credit card administrative fee is calculated from the total amount of the certificate issuing fee, postage, and online service fee. For more information on the total amount, please refer to the chart on the Credit Card Payment System: School Information page.)
  3.  The credit card holder may be someone other than the applicant. However, please enter the applicant’s information in the Basic Information Page.
  4. Please print the Credit Card Payment Complete page and mail it by post along with the application form.
  5. After completing the payment, cancellations and changes to the number of desired copies or postage (the destination, etc.) cannot be made. Furthermore, refunds are not possible. If additional certificates are required, please re-apply for the necessary documents (postage, online service fees and credit card administrative fees, etc., will apply separately).
  6. Credit card payment is not available for students who entered Keio University in or before 2008 and are applying for AUD (Architecture and Urban Design) Courses costing 800yen per certificate.

If the student is unable to pick up the certificate, a Letter of Authorization and a copy of identification of the proxy are required.
