▼ Applicable Faculties and Graduate Schools
Those who graduated from the Faculties of Letters, Economics, Law, and Business and Commerce; Graduate Schools of Letters, Economics, Law, Human Relations, and Business and Commerce; and the Law School and wish to obtain official academic certificates, must make a request to the Office of Student Services on Mita Campus. However, those who were enrolled in correspondence courses and those who were enrolled as non-degree students (non-degree students [for credit], non-degree students [non-credit], and research students) are not included.

 
▼ Those who were enrolled at Keio University as non-degree students (non-degree students [for credit], non-degree students [non-credit], and research students) should ask about getting certificates to the section in charge of academic affairs at the faculty or graduate school to which they were affiliated while they were enrolled. ▼ Protection of Personal Information
Keio University’s privacy policy requires us to confirm that those making the request are the persons in question. Therefore, we require those making a request to present an official form of identification. A proxy requesting a certificate must present his/her official identification, a copy of the principal’s official photo identification, and the Letter of Authorization signed by the principal.

Monday through Friday, 8:45 a.m. to 4:45 p.m. (The office is open as normal during the spring and summer breaks as well as during lunch time.)

Note: Certificates cannot be issued on Saturdays, Sundays, national holidays, school holiday (January 10), and during the university summer and winter break periods as the office is closed. Certificates  also cannot be issued during system maintenance and when other university events are taking place. These will be announced on our website.

▼Making a Request in Person

Those who graduated, completed, or withdrew from their program in or after March 1991 can visit the offices on the other campuses (Hiyoshi, Yagami, Shonan Fujisawa, Shinanomachi, and Shiba-Kyoritsu) to apply for and receive their certificates.
 
  • 400 yen per copy.
  • You will need to bring a form of photo identification issued by an official body. (See the section on Acceptable Forms of Identification.)
  • Fill out an application at Counter No. 1 (certification services) at the Office of Student Services, B1F, South School Building, Mita Campus. (Click here for campus map.)
  • It is possible to ask a proxy to request a certificate on your behalf. In this case, the proxy will need to present his/her official photo identification, a copy of your official photo identification, and the Letter of Authorization signed by you when making the request.

▼Making a Request by Mail from within Japan


  • Any requests made by telephone, fax, or e-mail will not be accepted.
  • It is possible to ask a proxy to request a certificate on your behalf. In this case, the proxy will need to send a copy of his/her official photo identification, a copy of your official photo identification, and the Letter of Authorization signed by you when making the request.   • Certificates will not be issued if the application and/or documents contain errors or are incomplete. If we cannot get in touch with you for three months from the day we receive the documents, all documents will be returned.
  • Only originals of the certificates will be issued. We will not send you a copy by fax or e-mail.
  • Be sure to send the following.

1. A copy of your official photo identification

See the section on Acceptable Forms of Identification.
The copy must be a clear, full size copy.

2. Application form

Download the following application form and fill it out. If you have trouble downloading the form, please prepare a document that clearly states the following.
(a) Your name in English (If you require Japanese certificates, we also need your name in kanji or katakana.) 
(b) Gender  
(c) Date of birth  
(d) Affiliation (faculty or graduate school, department, major, class)  
(e) Student ID number (Leave blank if unknown.)
(f)  Month and year of graduation, completion, or withdrawal
(g) Type of certificate (English or Japanese) and number of copies required  
(h) Whether the certificates should be sealed in envelopes and how (If necessary, give detailed instructions, e.g., which certificates should be placed in which envelope, how many certificates should be placed in each envelope.) 
      Note: Be sure you check with the persons you need to submit the certificates to before applying. We will not be able to seal certificates in envelopes once they have been sent to you.
(i)  Your contact details (home address, telephone number, and daytime contact details [mobile number, e-mail address, etc.])

3. Fee

  • 400 yen per copy.
  • We only accept money orders (郵便定額小為替, yūbin teigaku kogawase issued at any post office in Japan) or cash (Japanese yen only).
  • Do not fill out the Recipient (指定受取人) and Address and Name (おところ・おなまえ) fields on the money order. We may not be able to process it for payment if you fill out these fields. Please also note that we will not be able to accept money orders that were not issued in the past six months.
  • If you wish to send cash, please send it by registered mail.
  • We will not accept stamps as a form of payment.

4. Self-addressed stamped envelopes (We will not send the certificates to an “in care of” address.)

  • We will only send the certificates by post (standard mail). We will not send them by registered mail or courier.
  • All certificates are A4 in size.

Note:
(a)   Applications and payments for certificates must be made by campus, degree program, or affiliation. See the table on the guidance page of the e-shiharai.net website for more information. If you require certificates from more than one campus, apply separately to each campus, specifying the number of certificates you require each time. If you cannot select the number of certificates you require on the system, please call us or send us an e-mail. 
(b)   In addition to postage and the fee to issue certificates, the following service fees will be charged every time you make a payment through the e-shiharai.net website. 
(i)    System usage fee of 500 yen
(ii)   Credit card administrative fee (This fee is based on the total of the certificate issuance fee, postage, and system usage fee. Details of the fee can be found in a table on the guidance page of the e-shiharai.net website.)
(c)   The credit card holder may be someone other than the applicant. However, be sure to enter the applicant’s information on the personal details page.  
(d)   Print the page confirming fee payment by credit card and send it together with the application form. 
(e)   After completing payment, cancellations or changes to the number of copies required or postage (destination, etc.) cannot be made. Furthermore, payments made are non-refundable. If additional certificates are required, please re-apply. (Postage, system usage fee, credit card administrative fee, etc., will apply separately.).

5. Sending out the certificates

  • The certificates will be sent by EMS (Express Mail Service) to the address written in the Address field of the application. The EMS postage is charged together with the fee to issue certificates on the e-shiharai.net website.
  • The EMS rates can be found here.
  • We will use special EMS envelopes to send out your certificates so you do not need to provide self-addressed envelopes.
  • Write your address in block letters on the back of the application form if you need more space.
  • It is possible to have certificates sent to two destinations using one application if the destinations are in the same delivery zone. (Postage will be double.) If you wish to make use of this service, please write the addresses using block letters on a separate sheet.
  • Please call us or send us an email first if you need to have the certificates posted to two different delivery zones.
  • Only originals of the certificates will be issued. We will not send you a copy by fax or e-mail.
  • Please let us know if you require the EMS tracking numbers. We will send you the tracking numbers by e-mail after sending out the certificates.

▼Acceptable Forms of Identification

Driver's license, certificate of driving record (issued on or after April 1, 2012), passport, health insurance card, both sides of residence card (alien registration certificate), and basic resident registration card (with photo). Certificate of residence, employee ID card, and student ID card from another university are not accepted.

If you live overseas, your passport is the only acceptable form of identification.

▼Letter of Authorization (When Making a Request by Proxy)

  • The letter must clearly indicate the principal authorizing the proxy to make a request for and receive certificates on his/her behalf.

1.   Date
2.   Name (and seal or signature), date of birth, address, daytime contact number, and e-mail address of the principal
3.   Name, date of birth, address, daytime contact number, and e-mail address of the proxy
  • If the principal is living overseas,
-     a copy of his/her passport photo page must be submitted.
-     the principal's signature on the Letter of Authorization must be the same as that on his/her passport.

Q1: What kind of information is on a Transcript of Academic Record?
A1: A Transcript of Academic Record contains your name, date of birth, faculty/department/major, date of admission, date of graduation, degree received, courses taken, the academic year you earned credits (not your year level), number of credits, and grade.

Q2: Is it possible to get my GPA certified?
A2: The GPA system was not used at Keio University for students admitted before the 2016 academic year. Therefore the GPA cannot be provided to those students as an official score. For graduates of the Law school, the GPA will be shown in their Transcript of Academic Record, regardless of the year of admission to the university.

Q3: Is it possible to have my ranking in class certified?
A3: Keio University does not officially rank its students, except for the Law School. For all other faculties/graduate schools, “RANK IN CLASS IS NOT AVAILABLE AT KEIO UNIVERSITY” will be written on the Transcript of Academic Record.

Q4: Is it possible to have the Grading System certified?
A4: The Grading System is indicated on the reverse side of the Transcript of Academic Record.

Q5: I need to send my certificates directly to the overseas graduate school I am applying for. Can Keio University directly send the certificates to the overseas institution?
A5: Yes, it is possible as long as we are able to confirm that the certificates need to be sent directly from Keio University to the overseas institution. Please see here for the necessary documentation.

Q6: Can I find out the date of admission to and graduation from the undergraduate program?
A6: It differs by academic year. Please see below and obtain a Certificate of Graduation as necessary.
